Apply for a replacement COTA ID card today!

If you’ve lost your COTA (ID, Senior, Discount, Key) card, we can send you a new one. Simply submit your email below. A member of Team COTA will respond with a secure digital form for you to fill out. 

Once complete, we’ll mail you a replacement card.
