Sedan Voucher Program - Frequently Asked Questions

 
Question Will the Sedan Voucher Program replace Project Mainstream?
Answer No, the Sedan Voucher Program WILL NOT REPLACE Project Mainstream. The Sedan Voucher Program is a separate program, not affiliated with paratransit service that provides customers transportation options.
 
Question How can I obtain vouchers?
Answer Vouchers may be purchased in the following manner:
• In person at COTA Connection, 60 E. Broad St.
• By telephone with a credit card number – 228-1776
• By mail (check only) to COTA Connection (60 E. Broad St., Columbus, OH 43215) and received by the 15th of each month
 
Question How many vouchers do I need for a round-trip?
Answer You must use one voucher for each trip. Two vouchers are needed for a round trip.
 
Question How much will it cost for a Personal Care Attendant (PCA) to ride?
Answer A Personal Care Attendant (PCA) may ride at no charge.
 
Question How much will it cost for a child to ride?
Answer Children under the age of six (6) will not be charged. Children seven (7) and older must pay full fare for each ADA and non-ADA trip. Children under the age of four (4) must have a child safety seat. The customer must provide the appropriate safety seat.
 
Question How much will it cost for my service animal to ride with me?
Answer Service animals may ride with the customer at no charge.
 
Question How many vouchers may be obtained per month?
Answer Customers may receive up to ten (10) vouchers per month. Customers must use 50% of their vouchers before purchasing more. (Example: customer purchases 10 vouchers, 5 must be used.)
 
Question If I do not use all of my vouchers for the month, may I use them the next month?
Answer Yes, vouchers do not expire and are transferable if you wish to share your vouchers with other eligible customers.
 
Question What do I do if I lose my COTA ADA Identification card?
Answer Please contact COTA Connection, 60 E. Broad St., Columbus, OH 43215, at 228-1776. You will be required to pay a replacement fee of $10. Your COTA ADA ID card is required in order to use any Project Mainstream services.
 
Question How do I request a ride?
Answer Eligible customers may call Urban Express at 856-1000 or 1-877-840-0411 to schedule trips.
 
Question How soon can I begin calling for sedan rides?
Answer Upon receipt of the vouchers, trips may be scheduled.
 
Question What do I do if my vouchers are lost or stolen?
Answer Report lost or stolen vouchers immediately to the COTA Connection office. You should also contact the police. Unfortunately, replacement vouchers will not be provided.
 
Question What do I give the driver when I get in the vehicle?
Answer Customers are required to provide the driver with:
1) One Voucher
2) COTA ADA Identification card
 
Question Should the driver receive a tip?
Answer Providing a tip is optional. Tips represent gratitude for quality customer service. If your driver provides quality customer service, you may show your appreciation by giving a tip.
